The Service Tracking Application Set has applicability for Homeless/Public Assistance and Humanitarian/Disaster Relief, as well as other applications (e.g. corporate service tracking and even games). The main focus of this section is on Service Tracking for Homeless/Public Assistance and Humanitarian/Disaster Relief.
There are many different agencies and groups associated with providing public assistance (including clinics and hospitals). Services are not always well coordinated, and there is information that organizations/agencies need to share and coordinate, as well as efficiencies in paperwork and coverage by sharing information. This includes applications, proof of eligibility, reporting, referrals, and improvements in overall support.
The Application is similar to supporting individuals in a “Guardian” or “Call Center” Role and provides the following capabilities:

Having an Organization/Agency be the Primary Sponsor for the Individual (and be able to manage multiple individuals)

Allow other authorized Organizations/Agencies to read certain data, add information to service logs, and send comments to the Primary Sponsor for review and possible incorporation into the individuals profile

Allow Agencies to have data fields to track their requirements, plus customizable Services menus to help in tracking provided services

Ability to send secure referrals to other agencies

Role based Triggers/escalations (left or entered an area, needs assistance, hasn’t had contact in XX days, missed appointment, etc.)
